16 people share their stories about medication errors - insights and teaching

medication errors can have serious consequences for patients. Nevertheless, many people are not aware that an error can happen when taking medication. In the special edition "Talking about mistakes" of the "Pharmacies Umschau", 16 people shared their personal experiences with medication errors and tell what they learned from it. These stories come from pharmacies, hospitals, medical practices and families and show the individual challenges in dealing with medicines.
In many cases,medication errors can be avoided. However, there are still more than every fourth patient side effects that could have been avoided by a correct regulation. In 2 percent of cases, medication errors result in serious problems or even death. In order to prevent such mistakes, it is crucial to raise awareness of it and to establish an open culture of errors. Safety when taking medication can only be improved through a cross -subject prevention strategy.
The personal stories in the brochure illustrate the various facets of medication errors: from incorrectly prescribed doses to incompatible combinations to communication problems between patients and medical specialists. Each report helps to raise awareness of drug safety and to encourage people to talk about their experiences.
The "Pharmacies Umschau" has published a special edition entitled "Talking about errors", in which the 16 personal stories are presented. In addition, the current edition "7b/2024" is devoted to the medication errors and how they can be avoided. In the brochure there is also important information about the correct use of medication.
